Sylk Ova Hair Co, LLC, operating its medical services division as SOH Cranial Solutions, is committed to protecting the privacy and security of your Protected Health Information (PHI). We are required by law to maintain the confidentiality of your PHI, provide you with this Notice, and comply with the privacy practices described herein.

HOW WE MAY USE AND DISCLOSE YOUR PHI

We may use and disclose your PHI without your written authorization for the following purposes:

  • Treatment: To provide and manage your cranial prosthesis services, including consultations (virtual or in-person), measurements, and coordination with your referring physician, dermatologist, or oncologist.

  • Payment: To assist with billing and reimbursement. This includes submitting claims to insurance carriers, Medicaid, or Veterans Affairs programs under HCPCS codes such as A9282 and L8499.

  • Health Care Operations: For internal operations such as quality assurance, compliance monitoring, and maintaining our secure Microsoft 365 healthcare environment.

SPECIALLY PROTECTED INFORMATION (42 CFR PART 2)

Certain records, including Substance Use Disorder (SUD) treatment records, are subject to additional federal protections. We will not use or disclose your SUD records for any purpose (including litigation) without your specific written authorization, unless otherwise permitted by 42 CFR Part 2.

YOUR RIGHTS REGARDING YOUR PHI

You have the right to:

  • Access: Request an electronic or paper copy of your medical records.

  • Amend: Request correction of inaccurate or incomplete information.

  • Accounting of Disclosures: Request a list of certain disclosures of your PHI made over the past six (6) years.

  • Confidential Communications: Request communications in a specific manner or location.

  • Revoke Authorization: Revoke any written authorization for marketing or photos at any time in writing.

HIPAA SECURITY & ELECTRONIC DISCLOSURES

We implement administrative, physical, and technical safeguards to protect your PHI. Pursuant to Texas Law (HB 300), please be advised that your Protected Health Information is subject to electronic disclosure.
