Our Commitment to Your Privacy

SOH Cranial Solutions is required by law to maintain the privacy of your protected health information (PHI), provide you with this Notice of our legal duties and privacy practices, notify you following a breach of your unsecured PHI, and follow the terms of the Notice currently in effect.

How We Use Your Information

We may use and disclose your health information for treatment (coordinating your cranial prosthesis care), payment (verifying benefits, obtaining prior authorization, submitting claims), and health care operations (quality assessment, compliance, and business planning). We may also disclose information as required by law or for public health and safety purposes as permitted by HIPAA and Texas law.

Uses Requiring Your Authorization

We will obtain your written authorization before using your health information for marketing, the sale of your information, or any purpose not otherwise permitted under HIPAA. You may revoke any authorization in writing at any time.
